How about a modified Ban Xia Hou Po Tang ? For Nausea acupuncture may be the best bet, get her to come in 2-3x per week for 2 weeks and see if there isn't any difference.

How often are your Acupuncture treatments in the past?

Thanks for that! Yes, in this case I think you may be right, acu may be the best. I was treating her 2x/week, then down to 1x and just before this condition it was down to 1x/month, (with thai massage 1x/month).
The last acu we did she said she felt a bit more relaxed but no major difference? I will try to see her a couple times next week and see what happens.

I like the formula you suggested, but I wonder if her body is not responding to herbs at all??...again we'll see what progresses.

Although it is typically reserved for patients with strong constitution and/or excess pattern, you might consider a modified Da Chai Hu Tang with this patent. My thoughts are this: Some of what she is experiencing sounds like it might be related to her Gallbladder, i.e. right upper quadrant pain, nausea, acid (or bitter) taste in the mouth, sluggish bowel movements, alternating chills and fever,greasy/dirty tongue coating, etc.

Have you checked for Murphy's sign on her? Has she had an ultrasound of her gallbladder?

Because the stomach Qi needs to descend, (i.e. she is experiencing rebellious stomach Qi), it will be difficult for that to happen as long as she is constipated. Ban Xia Hou Pu Tang is definitely effective for some types of nausea - for example, I love this formula for first trimester morning sickness characterized by a nausea felt in the throat area - but my concern here would be that it is a slightly warming formula, and if her gallbladder is the issue, it might exacerbate her symptoms.

Maybe something along the lines of the following Rx might be efficacious:

Da Chai Hu Tang Jia Jian

Chai Hu 10
Ban Xia 10
Sheng Jiang 9
Huang Qin 6
Bai Shao 9
Zhi Shi 6
Da Zao 6
Da Huang 3-6
Xuan Fu Hua 6-10
Huang Lian 3
Zhu Ru 9
Xiang Fu 9
*You may add Dai Zhi Shi (15-30) to strongly subdue rebellious stomach Qi if the above works too slowly.

I know we are straying off topic here but are you suggesting that an acidic Environment is a yang excess while an alkaline environment is yin ?

is this possibly too simplistic of a diagnosis for nausea in this case or in any other case ?

From the diagnosis you have listed for metabolic acidosis such acidic pH, acidic taste in mouth, dry mouth, depression, digestive problems:

Simplistically, she is over acidified - too much Yang, and has to be alkalized to balance her back to yin alkaline state through alkaline ionized kangen water, and possibly with the addition of qi-healing and qigong..

In my humble opinion, all Oriental medical therapies such as qigong, qi-healing, acupuncture, herbs, acupressure and alkaline ionized water are alkalizing to restore the acid (yang) and alkaline (yin) balance.
